Summary
The UDS Table 9D Report supports Uniform Data System (UDS) reporting for health centers. It collects patient service revenue for a calendar year: charges, collections, adjustments, self-pay sliding fee discounts, and self-pay bad debt write-offs attributable to that year. You choose a UDS year (calendar year) and an as-of date. The as-of date reflects updates to that year’s charges made through that date (e.g., late postings or corrections). The report includes Raw UDS 9D (procedure-level and other detail for review or drill-down) and UDS 9D Report with the Official UDS 9D table (formatted for UDS reporting). You can exclude visit carrier codes (CPT codes); the UI provides a default list you can change. Use it to fulfill UDS Table 9D requirements and to reconcile UDS-year revenue and adjustments.Notes
- The report is driven by Year and As of Date only (no start/end date range).
- The UI will default the as of date to February 15th, the date the report is due.
- Exclude Visit Carrier Codes lets you remove specific CPT codes from the official table; a default list is provided. This exclusion only affects the rollup, not the raw report.
- Report is compliant to 2025 HRSA guidelines
